The Role of Coordinators in the DMN Programs

Coordinators are highly-trained employees who provide peer-to-peer disease management services to patients. This peer-to-peer relationship is at the heart of the DMN model. Because Coordinators are affected by the same condition and use the same medication to treat it as the patients they speak with, they are able to offer a unique level of understanding, empathy, and support. Coordinators contact patients on a regular basis to keep track of their condition-related health and ensure they are living the best quality of life possible while dealing with a rare or chronic disease. Coordinators offer guidance, provide education about their condition, discuss clinical research opportunities, and administer important quality of life questionnaires. The data they gather from these questionnaires informs our medical team’s clinical research work.
